Lucky for Life costs $2 per wager.
Fill in a Lucky for Life playslip by choosing 5 different "white" ball numbers from 1 to 48, and one "yellow" Lucky Ball number from 1 to 18. Or, ask for a Lucky for Life “Quick Pick” and let the Lottery terminal randomly pick your numbers.
Lucky for Life tickets cannot be voided; once printed there is no way to cancel them. Please be sure to check your ticket carefully after purchasing to make sure it is what you requested.

ADVANCE ACTION: Buy tickets for future Lucky for Life drawings with the ADVANCE ACTION option. Play up to 28 consecutive drawings in advance. When playing ADVANCE ACTION, fill out only one board (set of numbers) per playslip. (i.e. To play four sets of numbers in advance, fill out four separate playslips.)
REPLAY: To play your numbers again, hand the Lottery Retailer your existing Lucky for Life ticket and ask for a REPLAY. You will get a new Lucky for Life ticket with the same numbers and wager amount. Your new ticket will have the REPLAY “R” printed on it.

TOP PRIZE (5 Numbers + Lucky Ball)

• 1 top prize winning ticket: $365,000 a YEAR FOR LIFE (Annuity) or $5,750,000 (Cash Option)

• 2-14 top prize winning tickets: The prize value is split equally among the winning tickets; the minimum prize value for this category is $500 a week for life.

• 15 or more top prize winning tickets: The top prize liability of $7,125,000 is split equally among all top prize winning tickets and paid in one lump sum payment. The minimum prize value for this category shall not be less than any lower tier prize paid in that respective drawing.

Minimum top prize period is 20 years. Top prize winners may choose the Cash Option as an alternative to the Annuitized Payment Option. Top prize amount shown is based on one winning ticket. Top prize values governed by "split-prize" liability; the top prize is divided equally among the top prize winning tickets and may be less than shown.

SECOND PRIZE (5 Numbers)

• 1-20 second prize winning tickets: $25,000 A YEAR FOR LIFE (Annuity) or $390,000 (Cash Option)

• 21 or more second prize winning tickets: The second prize liability of $9,400,000 is split equally among all second prize winning tickets and paid in one lump sum payment. The minimum prize value shall not be less than any lower tier prize paid in that respective drawing.

Second prize amount shown is based on one to twenty winning tickets. THIRD PRIZE (4 Numbers + Lucky Ball)

• $5,000 prize value is guaranteed up to 1,000 winning tickets.

If the number of winning tickets exceeds 1,000 the total prize liability of $5,000,000 is divided equally among the winning tickets. The minimum prize value for this category is $200.

All prizes are rounded to the nearest whole dollar.

Special rules apply to winners that are not natural persons.

No, as long as the numbers drawn are listed on your ticket in a single row, it does not matter what order they are drawn. The Lucky Ball number, however, must match exactly.

You do not have to wait to cash in a winning Lucky for Life ticket when you play Advance Action. To claim your prize, present your ticket to any Lottery Retailer. They will cash your ticket, and give you an Exchange Ticket in place of your original ticket. Your Exchange Ticket will be valid for the remaining number of active drawings on the ticket.

Those who win the Top Prize or Second Prize have the option to receive a one-time cash lump sum payment in lieu of the lifetime annuity payments. "For Life" prizes are guaranteed for a minimum of 20 years. The lump sum option for the Top Prize is $5,750,000 (before taxes). The lump sum option for the Second Prize is $390,000 (before taxes).

If a Lucky for Life annuity winner who is a natural person passes away prior to reaching the guaranteed minimum prize, the estate, or beneficiary as directed by court order or other governing document, shall receive the remaining payments equal to the minimum guaranteed prize. If the annuitant dies during the annuity payment period, but after the minimum guaranteed prize has been paid, all payments shall stop.
